本片电影文学剧本曾经田汉修改润色。 南宋末年。一个初春的早晨，西湖旁边一家尚书府第内，礼部尚书姜华的独生女儿姜玉蕊，因读《剑南诗稿》而对当时朝廷的妥协偏安产生忧虑，同时也因她自己心无所托，不堪闺房寂寞，便与随身丫环翠霞同到花园赏春解闷。花园里的枫树上，独留下一片去年未曾落尽的红叶。玉蕊看见这样一片傲过风霜还是如此鲜红可爱的红叶，因而题诗红叶之上，付以流水，寄托情怀。阴险毒辣的皇帝御弟七王，不以国家社稷为重，苟且偷安，荒淫无耻，这时正带了妃嫔游湖取乐，恰巧看见了玉蕊，垂涎欲滴。已故兵部侍郎的公子文东和，考中状元后迟迟不见封官，心中抱负无从施展，闷闷不乐，这时也从湖中经过，拾得红叶，看见叶上诗句，赞叹不止。东和拾得题诗的红叶后，因赞赏题诗人诗才和忧国忧民的思想而思慕题诗佳人。一夜，正在挑灯夜读时，他又想起红叶上的诗句，睡梦中便遇见了仙姿窕窈、貌如玉蕊的少女，喜悦万分。醒来后便步韵和诗红叶之上，愿凭红叶去寻访题诗佳人，结为知音。文姜两府原是世交。姜华寿诞之日，东和与母亲同到姜府贺寿，得与玉蕊相遇。玉蕊见东和品貌出众，言谈不畏权贵，大胆指责七王，心中对东和的怀才不遇非常同情。东和虽尚未知题诗佳人就是玉蕊，但见玉蕊如此温柔大方，也异常高兴。七王有意于玉蕊，在姜华寿日送礼贺寿，姜华心中不安，前往外厅应付。姜华去后，东和在茶几上发现一小扇，见扇上字迹与红叶上诗句笔态如出一人之手，猜疑莫定。这时翠霞来找扇，东和从翠霞口中探知红叶上诗句乃是玉蕊所题，满怀高兴，立即恳求翠霞设法帮助与玉蕊相会。翠霞持扇送还玉蕊，告以红叶题诗为东和所拾。玉蕊心中正在暗暗欢喜时，翠霞便说东和今夜三更约往花园相会，玉蕊含羞应允。初夏的月夜，花好月圆，玉蕊与东和践约相会。东和将红叶交给玉蕊，玉蕊看了叶上的和诗，并听东和说出不愿在偏安之局随波逐流，惟恐妹妹受累等话，不但爱慕东和的才华，而且敬仰东和的为人。于是，两人志同道合，心心相印，互订终身，但愿相携相守，生死祸福，此心不变。文府托媒到姜府说亲，姜华刚接到聘礼，圣旨传来，七王奏选玉蕊为妃。玉蕊宁死不从，要求父亲上殿求情说理。姜华虽有爱玉之心，但觉得圣旨已下势难更改，而且畏惧七王威势，无可奈何，只有劝女顺从。媒人将玉蕊被七王强选为妃之事回报文府，文母听说，一时气昏，愁云惨雾笼罩整个文府。东和想起玉蕊的不幸与自己的遭遇，怒气填膺，立即动本参奏七王。玉蕊知事不可挽回，宁愿拼将一死以报东和而誓不进宫受辱，因命翠霞前往文府请东和来见一面。东和参奏七王后，自己来找玉蕊。两人见面时，朝廷文书传下，将东和状元除名，贬归琼州。东和见参奏七王无效，又劝玉蕊逃奔他乡。玉蕊自知无路可逃，便将一小盒赠东和，匆匆而去。东和打开小盒，见是一束春丝和题诗的红叶，知道玉蕊将为他殉情而死，大叫一声，匆匆追去。时已深秋，月缺花残，在玉蕊原来题诗之处，东和找到了玉蕊，两人难舍难分，痛苦万状。东和这时觉得玉蕊对爱情的忠贞不屈，而自己又遭贬谪，国恨家仇皆难伸雪，无颜独自偷生在人间，便与玉蕊双双投湖殉情，以死来控诉那荒淫残暴的七王对他们的无耻迫害。
