 在爱人神秘信件的召唤下，詹姆斯（杰瑞米·艾文 饰）重新回到了充满着秘密和诡谲的寂静岭。但当他踏入这座小镇的那一刻却开始了无尽极限逃生模式：永不停歇的防空警报撕裂耳膜，漫天余烬笼罩大地，熟悉的街道扭曲成噬人的迷宫，三角屠夫、无脸血腥护士、未知诡异生物无处不在，步步紧逼。 现实与幻象在寂静岭彻底崩塌，詹姆斯一边躲避致命追杀，一边试图用破碎的记忆拼凑出真相。

　　19世纪中叶，西班牙巴斯克地区一个偏远所在。衣着考究的阿尔弗雷德·奥提兹（Ramón Agirre 饰）来到了当地一个小镇，他自称为省政府工作，此行的目的是找当第一个外号“铁锤”的铁匠帕尔西·弗朗西斯科（坎迪多·乌兰加 Kandido Uranga 饰）了解事情。从当地 人口中得知，铁锤脾气暴躁，不好相处，他所在的铁匠铺已成废墟，传言中这个铁匠更被人们视作魔鬼。未过多久，小镇的男人弄清了奥提兹的真实意图，原来数年前的西班牙资产阶级革命后，传言卡洛斯的黄金就隐藏在铁匠铺中。利欲熏心的镇民和奥提兹前往铁匠铺，却不知这里正是地狱魔鬼出没的恐怖所在……

　　柳青（周星驰 饰）在一次偶然之中结识了戏班台柱花映红（林颖娴 饰），两人之间渐渐产生了情愫。青木（惠天赐 饰）因为修炼歪门邪道而走火入魔，打伤了师兄燕生（张翼 饰），祸害人间。青木觊觎花映红的美貌，处处迫害柳青和花映红两人，最终，柳青被青木害死，花映红的双眼也瞎了。
　　柳青对花映红的强烈感情感动了燕生，他施法将柳青的魂魄拉回阳界。柳青得知只要找到了慈光法体便可起死回生，而留给他的时间很短，只有七天。在这七天之内，柳青还得处处提防青木的迫害。最终，柳青还是错过了最后的期限，他不得不重返阴间和花映红天人两隔。

　　曼哈顿医院接收了一位年轻的女病人，据说她是被不知名的生物强暴了。医院的医生们想尽一切办法想把受害者身上奇怪的物质剔除出来，可是似乎怎么弄也没办法清除乾净。女受害者在半昏迷状态下自己离开了医院，她完全不知道她体内已经不是单纯的人类器官，衍生的物体将会给她最终的宿命.。
